Greening the Fleet - Urgent Community Response Teams

Urgent Community Response Teams support people and help them to stay at home with hte correct medication, support and equipment. having an OT and nurse or paramedic in a joint visit enables this to happen more effectively. Sue is an Advanced Clinical Practitioner at Southern Health NHS Foundation Trust, working in the Urgent Community Response Team (UCR) in North Hampshire. She is an occupational therapist by background, and is the Therapy Lead in her team. Within the team she has been part of the development of the integrated falls and frailty service, who were 2023 winners of the health hero award for best urgent care collaboration, and finalist at the HSJ awards.
Sue was awarded the 2023 Greener AHP award and AHP of the year for her work on the implementation of an electric urgent response van, which promotes healthy aging for older people living with frailty by reducing falls, long lies and avoidable admissions. This initiative reduced carbon emissions and saved approximately £200K in acute care costs.
